Discharging a Patient & Resolving Patient Care

When a patient no longer requires the care of the Community Team, they must be discharged from the Community app and their Patient Care record must be resolved.

To do so:

  1. From Patient Care, select the patient required.
  2. From the left-hand pane, select Discharge Patient.
  3. The Discharge Patient screen displays:

  1. Select an Outcome of Patient Care from the list available.
  2. Enter any additional text.
  3. Select Discharge.
  4. The Patient Care record is resolved and the patient is discharged from Community.

To view details of a discharge, from Patient Care , select the patient and then the Referrals screen, see Referrals Tab for details.
