Discharging a Patient & Resolving Patient Care
When a patient no longer requires the care of the Community Team, they must be discharged from the Community app and their Patient Care record must be resolved.
To do so:
- From Patient Care, select the patient required.
- From the left-hand pane, select Discharge Patient.
- The Discharge Patient screen displays:
- Select an Outcome of Patient Care from the list available.
- Enter any additional text.
- Select Discharge.
- The Patient Care record is resolved and the patient is discharged from Community.
To view details of a discharge, from Patient Care , select the patient and then the Referrals screen, see Referrals Tab for details.
